ABSTRACT

In this retrospective study, we present our experience using open Küntscher nailing (K-nailing) which is still performed in developing countries for femoral fractures. Of 157 acute fractures treated between January 2003 and December 2009, 100 were stable (63.7%) and 135 were located within the middle third of the shaft (86%). Comminution was absent or minimal in 135 (86%) cases. Fracture union was achieved at an average of 14 weeks (range, 10 - 25). The duration of follow-up was 17 months (range, 6 - 36). Final evaluation showed that 129 (82.2%) patients had a good result according to modified Kempf’s criteria. The predominant complications were infection (n=5; 3.2%), nonunion (n=9;5.5%), implant failure (n=11; 7%), and malunion (n=27; 17.3%). Open reduction and fixation with a tight fitting Knail can give good results in selected acute femoral shaft fractures but knowledge of potential complications is needed.

ABSTRACT

Authors report the results of the therapeutic charge of 126 children and teenagers, with serious traumatism, during physical and sporting education section in Abidjan. It was about children and teenagers, from 6 to 19 years old, pupils in the various public and private schools of Abidjan's city and its suburbs. The average age is 14 years old, and the repartition according to the sex emphasizes a clear male prevalence [90 cases]. The indictment sporting disciplines are so many and varied, dominated by football [54 cases]. It is however advisable to underline that certain sports activities [fencing, fight, horsemanship, judo, etc.] are found only in some private establishments. At the anatomo-pathological level, the lesions met are multiple, from serious ankle sprain to complex bones fractures. The most and essential therapeutic indications result from the injury type and the displacements size or extent. We thus carried out 41 surgical processing against 85 orthopaedic processing. As a whole, the results carry up are very satisfactory

ABSTRACT

The association of a fracture-avulsion of the anterior tibial tubercle, with avulsion of the patellar ligament is exceptional. The authors report the observations of 3 adolescents with average age is 14 years old, having presented this lesion association during a sporting casualty [football, basketball, and jump]. In the 3 cases, it about lesion of IIIc state of Frankl, which has occurred at the longilineals subjects, without antecedent of disease of Osgood-Schlatter. The processing was surgical: fixing of the anterior tubercle by transversal-bones points for 2 patients and by 2 posterior former screws pressed on disc for the third patient, reintegration of the patellar ligament by transversal-bone points. At the end of the intervention, a crural-foot plaster in extension was made during 4 to 5 weeks. The favourable evolution for 3 patients was marked by the consolidation by epiphysiodesis of the anterior tibial tubercle, after 5 months of evolution. The results obtained can be regarded as very good, in spite of the deficit of extension of 10° observed at one of the patients
